This includes research on the chemical makeup, biological processes, physiological and behavioral impacts, mechanisms of action, and both therapeutic and non-therapeutic applications of pharmaceuticals.<\/p>\n<p><strong>Pharmacology is broadly categorized into two terms:<\/strong><\/p>\n<p><img decoding=\"async\" class=\"zn-img-reponsive lazyload\" data-src=\"https:\/\/www.diginerve.com\/wp-content\/uploads\/2022\/09\/pharma-terms.png\" alt=\"Pharma Terms\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" \/><\/p>\n<p>&nbsp;<\/p>\n<h2><strong>PHARMACOKINETICS IN PHARMACOLOGY<\/strong><\/h2>\n<p>The term \u2018Pharmacokinetics\u2019 is derived from ancient Greek words, <em>pharmakon<\/em> meaning \u2018drug\u2019 and <em>kinetikos <\/em>meaning \u2018moving\u2019 or put in motion. Pharmacokinetics is a journey of a drug through the body, and in this, a drug passes through four different phases- Absorption, Distribution, Metabolism, and Excretion, which are called ADME properties<em>.<\/em><\/p>\n<p>The selection and modification of drug-dose schedules are aided by pharmacokinetic parameters.<\/p>\n<p>Let\u2019s discuss the pharmacokinetic parameters:<\/p>\n<ol>\n<li><strong><u>ABSORPTION:<\/u><\/strong><\/li>\n<\/ol>\n<p>Absorption is the movement of a drug to the site of action from the site of administration. Various factors affect the extent and rate of absorption of the drug, such as routes of drug administration, dosage form, physiochemical properties of the drug,\u00a0 bioavailability, pharmacogenetic factors, etc.<\/p>\n<p><img decoding=\"async\" class=\"zn-img-reponsive lazyload\" data-src=\"https:\/\/www.diginerve.com\/wp-content\/uploads\/2022\/09\/2.png\" alt=\"\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" \/><\/p>\n<p>First Pass Metabolism: It is a phenomenon in which a drug is metabolized or transformed at a specific site before the drug reaches its site of action or systemic circulation, resulting in a lower concentration of the active substance. The principal site of drug metabolism\u00a0is the liver.<\/p>\n<p>Enzymes responsible for drug metabolism are classified into two categories:<\/p>\n<ul>\n<li>Microsomal enzymes: They are present in the smooth endoplasmic reticulum of the liver, kidney, and GIT. Example: cytochrome P450.<\/li>\n<li>Non-microsomal enzymes: They are present in the cytoplasm, and mitochondria of different organs. Examples: esterase, amidase, and hydrolase.<\/li>\n<\/ul>\n<p>Two Phases of Metabolism: Phase I metabolism converts a parent drug to polar active metabolites, via., oxidation, reduction, and hydrolysis reactions, while phase II metabolism converts a parent drug to polar inactive metabolites including conjugation reactions.<\/p>\n<p><strong>Watch this video of Pharmacology for UnderGrads and gain an in-depth understanding of Pharmacology in MBBS.<\/strong><\/p>\n<div class=\"responsive-video\"><iframe title=\"YouTube video player\" src=\"https:\/\/www.youtube.com\/embed\/bUef5t9VB0Y\" frameborder=\"0\" allowfullscreen=\"allowfullscreen\"><span data-mce-type=\"bookmark\" style=\"display: inline-block; width: 0px; overflow: hidden; line-height: 0;\" class=\"mce_SELRES_start\">\ufeff<\/span><span data-mce-type=\"bookmark\" style=\"display: inline-block; width: 0px; overflow: hidden; line-height: 0;\" class=\"mce_SELRES_start video\">\ufeff<\/span><\/iframe><\/div>\n<p>Drug excretion refers to the removal of drugs in unchanged or modified form out of the body.<\/p>\n<p>The major route of excretion:<\/p>\n<ol>\n<li>Renal excretion: It involves three major physiological processes, namely, glomerular filtration, active tubular secretion, and passive tubular reabsorption.<\/li>\n<li>Hepatobiliary excretion: The drug with a molecular weight of more than 300 Da and polar drugs are excreted in the bile.<\/li>\n<li>Gastrointestinal excretion: After the oral administration of a drug, a part of it is not absorbed and excreted in the feces.<\/li>\n<li>Pulmonary excretion: Volatile drugs, such as gaseous anesthetics, are excreted via the lungs into expired air.<\/li>\n<\/ol>\n<p>Saliva, sweat, tears, breast milk, vaginal fluid, nails, and hair are considered the minor routes of drug excretion.<\/p>\n<h3><strong>VARIOUS PHARMACOKINETIC PARAMETERS:<\/strong><\/h3>\n<ol>\n<li><u>Half-life:<\/u> It is the time taken for the drug concentration in blood or plasma to reduce to half of the original amount, i.e. the amount of the drug in the body is reduced by 50%. A drug&#8217;s half-life varies from patient to patient because of a variety of patient and drug-specific factors.<\/li>\n<\/ol>\n<p>The formula for the half-life is <strong>t\u00bd = 0.693 \u00d7 V<sub>d<\/sub> \/CL<\/strong> where,<\/p>\n<p>V<sub>d <\/sub>is the volume of distribution and CL is a clearance factor.<\/p>\n<p>2. <u>Order of Kinetics:<\/u> For many medications, the most typical is First order kinetics, meaning that a fixed proportion of the drug is eliminated from the body at regular intervals of time. Only a few medications, like ethanol and phenytoin, undergo zero order kinetics wherein, a fixed quantity of the medicine is eliminated after each period.<\/p>\n<p>3. <u>Clearance of a Drug:<\/u> It is the volume of plasma cleared or removed out of the drug by hepatic and\/or renal excretion, among other organs.<\/p>\n<p>Clearance =\u00a0\u00a0\u00a0 Rate of Elimination<\/p>\n<p>Plasma drug concentration<\/p>\n<p>And, Total clearance is calculated by <strong>Cl<sub>t <\/sub>= Cl<sub>h<\/sub> + Cl<sub>r <\/sub>+ Cl<sub>others <\/sub><\/strong>where,<\/p>\n<p>Cl<sub>t<\/sub> = Total clearance<\/p>\n<p>Cl<sub>h<\/sub> = Hepatic clearance<\/p>\n<p>Cl<sub>r <\/sub>= Renal clearance<\/p>\n<p>Cl<sub>other <\/sub>= Clearance from all other routes<\/p>\n<p>4. <u>Steady-state plasma concentration:<\/u> When a drug dose is administered regularly over a certain time, a steady state is reached. It is a point where the amount of medication absorbed and the amount removed from the body is in equilibrium.
